About the role:

DSA are looking for skilled health and aged care professionals to join our Intake team as a Triage Consultant.

The role:

A rewarding opportunity for a passionate Dementia professional to join the Dementia Support Australia team at HammondCare on a full-time basis up until June 2028, with flexibility to work at any of our Sydney, Melbourne or Perth sites.

The Triage Consultant works with families, health services and care providers to determine risk and service level for new DSA referrals, and provides short-term advice by phone and email, to manage changes in behaviour related to dementia.

This role works closely with different services within DSA to reduce the impact of these behaviour changes on people living with dementia, their carers and staff.

What do we need from you?

  • Experience in Australian health or aged care (essential). 
  • Extensive knowledge of the types of dementia, their presentation, and strategies to manage behaviour changes.  
  • High level of negotiation, communication and relationship building skills
  • High attention to detail and accuracy
  • Great written communication and report-writing skills.  
  • Able to prioritise multiple tasks and meet deadlines. 
  • Comfortable working with databases and online client management systems.  
  • Qualifications in Allied Health, Nursing, Aged Care or Mental Health (highly desirable).
