Dementia Support Australia, in collaboration with the Wicking Dementia Research and Education Centre, delivers the Dementia Respite Education and Mentoring (DREAM) project to strengthen workforce capacity in respite dementia care. The DREAM Dementia Support Coach plays a distinctive role in working alongside staff within community organisations and residential aged care settings. This is achieved by face-to-face engagement, phone or video-based support, observation, modelling of practice, and reflective dialogue. Coaches deliver tailored education and mentoring based on the core educational modules of the DREAM project. The DREAM Dementia Support Coach designs and implements coaching content to present to staff groups and coaching of individuals. The DREAM Dementia Support Coach liaises and negotiates with staff, service leaders, and other stakeholders to ensure coaching is collaborative, contextually appropriate, and aligned with the needs of each setting. The role requires a high level of professional autonomy, adaptability, and initiative, along with a commitment to person-centred and psychosocial approaches to dementia support.


This role is offered as a Fixed Term Contract until June 2026. This role is based out of our Daw Park office.

Responsibilities

  • Dementia Support Coach will facilitate on the ground support, implementation of relationship-based support strategies, modelling and coaching with clients, carers and care staff.
  • Work collaboratively with peers to develop the skills, expertise and/or applied knowledge of aged care staff and carers to understand the person living with dementia and develop best practice support and care.
  • Support the implementation of the recommendation plans on the ground, drawing on modelling and coaching techniques, and seek guidance from Team Leader, Dementia Consultant and Senior Consultant as required.
  • Facilitate and support the trial and implementation of brokerage items with clients and their carers, and seek guidance from Team Leader, Dementia Consultant and Senior Consultant as required.

What do we need from you?

  • Strong organisational abilities, including effective time management and ability to prioritise in a busy work environment and competing demands.
  • The ability to work both independently and as part of a team.
  • Demonstrated commitment to excellence in customer service principles including conflict resolution to ensure the consistent provision of exceptional service to internal and external customers
  • A current driver’s licence and safe driving record
  • Willingness to travel and work intrastate and interstate for up to five days at a time to support the delivery of DC services.
  • Willingness to work flexible hours at times and as required to meet the assessed needs of clients and carers.
  • Qualification in relevant field; Allied Health assistant, Diversional Therapy,  Enrolled Nursing (EN), Aged Care, Cert III or IV in Individual Support / Leisure and Health or similar highly desirable
