The role

The Team Leader ensures that the Dementia Support Australia (DSA) service is tailored to the needs of individual clients and ensures the service delivers on its key performance indicators.

This role is offered as a Temporary Full Time or secondment opportunity until July 2028. This role will be supporting our Tasmania HACDSP Team. 

Responsibilities

  • Provide leadership to a multidisciplinary team of staff across a possible three locations ( Hobart, Launceston and then supporting referrals from the North West of Tasmania) with a specific focus on the DSA Program, in accordance with our policies, procedures and HammondCare values.
  • Work as part of an integrated team to ensure a nationally consistent approach to our ‘‘Next Chapter’
  • Develop a culture of empowerment, mentorship and support to the designated team of DC Consultants, Dementia Support Coaches and Family Liaison Officer roles ensuring their wellbeing and safety is considered
  • Oversight of individual and team to ensure DSA Service Delivery operates within the case management framework is of the highest quality to reduce the impact of behaviours on people living with dementia, their carers and staff
  • Working in partnership with other Team Leaders to meet the needs of the broader DSA Program and be part of the solutions to any broader program deliverables
  • Contribute to ensuring the wider DSA Program meets its administrative, financial, risk and quality and reporting goals across the programs reporting KPI’s
  • Provide direct consultation and support to our clients as required;
  • Ensuring timely allocation of incoming referrals to the appropriate DSA staff member in alignment with DSA KPI’s
  • Provide regular reporting to the Regional Manager on all staff performance, service quality, operational performances and issues
  • Recruit Dementia Consultants, Dementia support coaches and Family Liaison Officers with appropriate skills for the relevant role
  • Key stakeholder management in the start up, and ongoing, through effective strategic conversations with local managers and staff in residential aged care, home care and the hospital settings to ensure that the program is understood and that key stakeholder management is effective
  • Effectively manage customer complaints or challenging situations as they arise, with early notification to the Regional Manager or the National Program Manager when needed
  • Work with Regional Manager to address incidents and feedback relating to the quality of the DSA Service
  • Improving your own knowledge and skills as part of the Learning and Dementia Centre team
  • Working in partnership with the other Team Leaders to ensure the DC office environment is safe, including routine maintenance and functioning equipment and ICT
  • Working in partnership with the other Team Leaders to oversees local use and maintenance of DC fleet vehicles including utilisation, electronic tracking and reporting
  • Responsible to delegated approvals for brokerage and travel, in line with financial delegation and HammondCare Policy

What do we need from you?

  • Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action
  • Tertiary qualifications in Nursing/ Allied Health or health- related degree
  • Demonstrated experience and knowledge in supporting older people and carers living with dementia, with particular emphasis on psycho-social approaches to case management
  • Ability to manage and support a team of skilled professionals across multiple sites and locations
  • Ability to work on multiple tasks and projects simultaneously
  • High level communication and interpersonal skills
  • Strong planning and organising skills with the ability to prioritise tasks to meet deadlines
  • Strong analytical skills and attention to detail
  • Good customer management skills for delivery of services to HammondCare and external customers
  • Strong administrative and computer skills
  • Willingness to travel intra and interstate

Desirable

  • Eligibility for registration as a Health Practitioner in Australia
  • Evidence of study and or qualifications in dementia
