The opportunity
The Admissions Coordinator will work with internal and external stakeholders to facilitate a streamlined and timely process to assist in understanding resident needs, goals, and preferences and admission to services which best meet their needs.
In partnership with the Service Managers, this role will network externally ensuring that HammondCare’s services are well known in the community, liaising with local GP’s, home care services (including HammondCare’s Home Care Services), hospitals and other potential client sources to ensure a sales pipeline of referrals are made and that a waitlist is maintained.
This is a permanent full-time role based at our Wahroonga residential site. The role may require travel to our Turramurra, Woy Woy and Erina sites on an as needs basis.
The successful candidate may work out of our Woy Woy or Erina sites however travel would be required weekly to Wahroonga and/or Turramurra.

Responsibilities
Internal and External Communication of HammondCare Services
- Establish effective communication with prospective residents and their representatives
- Establish effective relationships within the local team
- Promotion and establishing close relationships, regular networking with other HammondCare services and external related organisations in local clinical and social networks to facilitate clear referral pathways
- Participate within a continuous quality improvement team environment
Admission process
- Review occupancy levels and maintain targeted occupancy rates by actively managing the waiting list
- Understand the financial implications of vacancies and meeting occupancy targets
- Manage enquiries and telephone calls for services within the area
- Ensure prospective residents and their representative are given a tour and explanation of HammondCare’s Philosophy of Care and profiling process
- Establish general suitability for the prospective resident. If suitable, liaise with Residential Home Manager to complete the Care Planning Assessment Tool (CPAT)
- Ensure all pre-admission documentation is signed and returned, in a timely manner
- Actively manage service waiting lists by maintaining and nurturing relationships with networks and residents on waitlists, providing regular updates throughout the sales funnel to service admission
- Establish client agreements where residents and their representative clearly understand services and associated financial costs prior to admission
- Update and maintain Salesforce and waitlists on an ongoing basis
- Daily monitoring and weekly reporting of occupancy
Who are you?
- Commitment to engage and align with HammondCare’s Christian Mission, Motivation and Mission in Action
- At least 3 year’s demonstrated experience in a busy administration role
- Experience working in a sales capacity
- Excellent communication skills, both written and oral
- The ability to engage with external stakeholders and develop relationships to better promote HammondCare
- Ability to work independently and as part of a team
- Empathy and patience
- Driver’s licence and ability to travel to network with potential referrers
- Experience and knowledge of Aged Care fees and charges and be able to apply this to the preparation of resident agreements - desirable
- Financial acumen desirable
- Salesforce experience desirable
