About the role: A rewarding opportunity for a passionate Dementia professional to join the Dementia Centre Team at HammondCare on a Permanent, Full-Time at our Daw Park site.
The role focuses on improving the lives of individuals with dementia across HammondCare services by building staff capacity to deliver best practice care. The Dementia Centre Advisor will support staff at all levels in implementing HammondCare’s Model of Care and Case Management framework. As a Dementia Centre Advisor, you will engage in projects and contribute to knowledge generation on best practice dementia care. You are capable of working both autonomously and as part of a team.
Responsibilities - What do we need you to carry out:
- Understand and engage with HammondCare Mission in Action, Model of Care, Case Management Framework and practice guidelines, policies and procedures.
- Providing dementia specific expert advisory services within HammondCare services, through participation in regular clinical review meeting and on the ground support across all cottages for the site allocated.
- Be expert in and utilise the HammondCare Model of Care, Case Management Framework and practice guidelines, policies and procedures are adhered to in individual client/cottage consulting work.
- Build staff capacity through communication, education, mentoring and coaching in practice, this can include spending time with the team on the ground, conducting case study discussions and review of documentations (e.g. CPAT, BSPs etc.).
- Plan engagement with individuals and teams to achieve agreed practice development objectives and quality improvements.
- Provide support through mentoring and modelling in the SDCP onsite (where applicable) and be active part of CRT and CAC. This support can extend to other HC SDCPs as needed.
- Access current evidence-based guidance and knowledge, ensure it is aligned with the Dementia Centre and utilise to inform all activities in the role.
- Utilise agreed HammondCare and the Dementia Centre processes to communicate, monitor and manage work delegations and projects including evaluation and reporting.
- Participate, contribute and learn as part of the Dementia Centre team in team meetings, professional supervision and team activities.
- Work on other projects and assignments as required and in line with areas of skill, expertise and knowledge.
What do we need from you?
- A minimum of 3 years’ experience in the Australian Aged Care Industry AND able to demonstrate currency in your field of expertise
- Registered Nurse or Allied Health qualification, with further education in Certificate IV in Training and Assessment (TAE40116 or TAE40110), including TAEASS502 and TAELLN411 or higher, is highly desirable.
- Demonstrated skills in teaching, mentoring and coaching others
- Knowledge and skills in relation to adult learners and development of learning resources
- High level of negotiation, communication and relationship building skills
- Ability to prioritise multiple tasks and meet deadlines
- Problem solving and decision-making skills relevant to the aged and dementia care context
- Strong interpersonal and communications skills both written and verbal.
- A current drivers licence and safe driving record.
- Willingness to travel between HammondCare’s sites and services and across Australia
- Strong administrative and computer skills using Microsoft Office products such as Outlook, Word, Excel, PowerPoint and Teams.
- Ability to work independently and as part of a team.
